Insurance
Chatsworth Dental Group is a fee for service dental office. We accept dental indemnity insurance plans that pay a portion of the dental fees of the office.  We serve PPO patients as an out-of- network provider. We cannot care for HMO patients as their contract requires that they be treated by specific dental offices.

Payment
We bill as services are rendered. For patients who pay in advance of a long term treatment plan, a five percent discount applies.

Payment Plans
Individual payment plans may be arranged with our office for long term treatment.

Cancellation of Appointments
We require a minimum of 24 hours notice for cancellations so our staff can offer time to other patients. A $50 fee will be charged should a patient give insufficient notice.
